Wondering how to set up an appointment with our Dietitians?
-
Doctor referral & self-pay options are available. Ask your Doctor at your next appointment about Dietitian referral options or contact our Dietitians about self-pay appointments.
-
All outpatient visits start with an initial appointment to review labs, intakes, medical history, & more. Follow-up appointments are then set up on a weekly, monthly, or bi-monthly basis.
-
All inpatients are seen once admitted to SMC if found at nutritional risk. Care is coordinated with the health care team and all patients are regularly followed up on to monitor progress.
